Step 1 - Register
Please complete the registration form for each student using the "Register" button below.
Step 2 - Sign-up with ADE
For those using the ESA, Empowerment Scholarship Account, this will be done on the Arizona Department of Education website, www.azed.gov/esa
Please make sure you have all the items needed to apply, Student's Birth Certificate: a full-color image, both sides of the child's birth certificate to verify parent/guardian status and age (students must be at least 4 years old), Proof of Arizona Residency: A physical address verification in the parent or guardian's name (P.O. Boxes are not accepted). Acceptable documents include:
A utility bill (water, electric, gas, cable, or internet) issued within the last 60 days (must show all four corners of all pages).
A current bank statement or W-2/payroll stub.
A valid residential lease agreement or mortgage statement.
Note: If you do not have proof of residence in your name, you can submit a notarized Affidavit of Shared Residence Form. (I can also notorize this form for you.)
And if applicable special education documentation (including the last page, signature page). If you need assistance, please ask.
Step 3 - Attend Orientation
Parents and students will attend an orientation to get signed into all programs and to go over policies, and procedures.
